Who may avail of the Service?

1.      Kinder age                  -           (4 ½ - years old)

2.      Elementary age         -           (6-11 years old)

 

Schedule of Availability of Service:

1.      Kinder and Elementary: two (2) weeks before the start of the academic  school year

 

Requirements for Enrollee:

1.      Kinder and Elementary

*      Report card for Grade II-VI pupils/students

*      Birth Certificate (original/xerox copy) for Kinder and Grade I pupils

2.      Transferees

*      Certificate of good moral character

*      Forms 137 and 138

*      Reading ability assessment/Placement test

  

RECRUITMENT, EVALUATION AND SELECTION OF TEACHER IN PUBLIC ELEMENTARY SCHOOL

 

Who may avail of the Service?

*      Applicants who have valid certificate of registration/professional license as a teacher from a Professional Regulation Commission (PRC) seeking to be recruited as a teacher.

*      Qualified applicant who has a valid professional license issued by the Philippine Regulation Commission and who meets the requirements of the school evaluation board.

*      Bonafide resident of the barangay municipality, city or province in the Personal Data Sheet

*      List of candidates who obtained an over score of 50 points and above based on the criteria provided in the hiring guidelines during the evaluation and screening by a School Evaluation Board.

2. CSC Forms 212 (revised 2005) in 2 copies with at least 2x2 ID photo

3. Certified photocopy of PRC certificate of registration/license

4. Certified photocopy of ratings obtained in Licensure Examination for Teachers/

Professional Board Exam for Teachers

5. Certified copies of Transcript of records foe baccalaureate course

6. Copies of service record performance rating and school clearance for those

with teaching experience

7. Certificate of specialized trainings

6.       Consolidate individual ratings and submit initial rank list of qualified applicants to the District Selection Committee

7.       Secure Copies of RQA for posting and information disseminations.
